An update as to where the state is on the new revalidation for provider process.  At this time, if you have an active number and only need to revalidate, DHS will be sending letters out according to counties and ask that you do not try to revalidate your district Medicaid provider numbers until you receive your letter which will give a 90 day notice.

If you need to reactivate or apply for a new Medicaid provider number for your district, please contact us and we will work with DHS and your district on this process.  There will be a fee for providers, but will exclude Therapy (OT, PT, Speech) and Audiology, all other providers will have to pay a fee when they revalidate or activate a number, such as vision/hearing, school based mental health, and personal care.
